WAIVER FORM

In consideration of and through my involvement in this Nantahala Racing Club Training Camp.

I acknowledge and agree that:

1. I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their actions, inactions, or negligence of, but the actions, inactions or negligence of others, the rules of play, or the conditions of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

2. I knowingly and freely assume all risks and accept personal responsibility for the damages following such injury, disability, or death.

3. I, for myself, and on behalf of my heirs, assigns, and next of kin, hereby release, hold harmless and promise not to sue the American Canoe Association, the USACK, the Nantahala Racing Club, the Nantahala Outdoor Center, their board of directors, officers, agents, sponsors, and/or employees, with respect to any and all such injury, paralysis, dismemberment, death and/or loss or damage (except that which is resultant of gross negligence and/or willful or wanton misconduct).

MINOR WAIVER AND RELEASE

1. I acknowledge and fully understand that the above stated minor camp participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from his/her actions, inactions, or negligence of, but the actions, inactions or negligence of others, the rules of play, or the conditions of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

2. I also understand that the minor under my care and guidance will only be supervised during the scheduled workouts and will not hold accountable the above for problems occurring during the non-supervised times during the training camp.

3. I acknowledge that I am the parent or legal guardian of the participating minor.
